Privacy Commitment

Cancer Genetic Risk Assessment Questionnaire CancerGene Connect (CGC) was designed to reduce the duplication of medical histories taken by many of the physicians you may encounter in the course of your care. The family history and medical information that is entered is used in risk calculations to determine your personal risk to develop cancer and/or have an inherited genetic mutation. CGC is part of our clinical database. We are committed to protecting the privacy of our patients. We will use our best efforts to ensure that the information you submit to us remains private, and is used only for the purposes as set forth herein. The following reflects our commitment to you. This notice applies to all information collected or submitted on this secure and non-public website.

General Information Disclosure Except as described in this privacy policy, we do not disclose information about the personal information that you provide, such as your name, address, e-mail address, telephone number, personal or family medical history, etc., to any outside parties, except when it is required by law or to protect our users or others. But, we may record and share aggregated information with other physicians and medical personnel who are involved in your care. Also, we may employ other individuals to perform functions on our behalf. Examples may include hosting our web servers, analyzing data, updating our clinical database, and providing customer service. These other individuals will have access to your personal information as necessary to perform their functions, but they may not share that information with any third party. Our institutional policies prohibit the release or disclosure of any individuals' information or the use of any confidential health information other than when permitted by law.

Updating Questionnaire Information CGC allows you to change or correct your personal health information at any time. To do so, simply log onto the CancerGene Connect website and start to enter or modify your information. If you wish to take a break and return the website at a later time, simply close the website, and you will be able to use your CGC number and password to access and edit the information that you have already entered.
If you decline to enter this information online, a paper version of this questionnaire will be given to you at the time of your appointment to complete. Your medical and family history information is needed to provide you with an accurate genetic risk assessment.

Privacy Commitment Changes If we decide to change our privacy commitment for CGC, we will post those changes here so that you will always know what information we gather, how we might use that information, and whether we will disclose it to anyone.
